SCHOLARSHIP AWARDS
Instructions for Applicant

1. Application forms are available upon request in the GSBS Dean’s Office, 4.429 Levin Hall, or online.

2. Mail one original and EIGHT copies of the completed application (Part I) and attachments (copy of first page of each published work is sufficient) to the Scholarship Committee, GSBS, 4.429 Levin Hall, Route 1050. Deadline for receipt is August1, 2007. NOTE: It is not sufficient to attach a C.V. covering the information requested. An application form must be submitted.

3. Forward Part II "Reference Request Forms" to your mentor.. Allow sufficient lead-time so the completed forms will be received by the GSBS office on or before the August 1, 2007 deadline.

4. Request that one copy of your official UTMB academic transcript be mailed directly from the institution to GSBS and marked for the attention of the Scholarship Committee, GSBS, 4.429 Levin Hall, Rt. 1050. The deadline for receipt is August 1, 2007.

5. To be eligible for scholarships for which financial need is a criterion, students must have this need officially determined by the university’s Student Financial Aid Office (1.206 Old Red; x 24955) Financial need eligibility must be established for each academic year, regardless of past applications and qualifications. To ensure that all financial need decisions are completed by the scholarship application’s April deadline, applications for a financial need determination must be completed by August 1, 2007. Please contact Enrollment Services regarding financial need.

6. Applications will be judged in 5 categories of equal weighting. These categories are:

   1. academic performance (grades)

   2. quality of personal statement and research experience descriptions

   3. letters of recommendation

   4. publications generated while a graduate student (judged appropriate to the years of student experience), and

   5. other indications of potential, leadership, service, or worthiness that do not fit into the above categories.

NOTE: By submitting one original and EIGHT (8) copies of the application form, students will automatically be considered for ALL scholarships for which they qualify. Applicants are responsible for complete applications reaching the GSBS on schedule. Applications received after August 1, 2007, and incomplete applications WILL NOT be accepted.

SUCCESSFUL APPLICANTS WILL BE NOTIFIED NO LATER THAN SEPTEMBER 5, 2007.

ACCEPTANCE OF THE SCHOLARSHIP MUST BE MADE, IN WRITING, WITHIN ONE WEEK OF THE STUDENT’S NOTIFICATION OF THE AWARD.
